Baptism Booking Form for a Child

If you would like to speak to us about having your child baptised, please contact the vicar, who would be delighted to come and start exploring this with you. There is some preparation to go through before a baptism can be arranged, so please make contact in plenty of time.

Once the vicar has agreed a date with your for the baptism, please complete the form below. This will give us the details we need to organise a service of baptism for your child.

Please specify the given surname that you wish to have recorded in the church's baptism register.
Please specify all Christian names (first names) for the child, including any middle names
Please give the full postal address for the child (where they live) at the time of their baptism. Please enter the full address, as this website does not have the facility to look this up using your postcode.
The Baptism
Which church is this baptism going to take place in?
Most people fill this form in once a date has been agreed; please indicate the date we have agreed to baptism the child.
Your Details
We need certain details of the child's parents in order to record a baptism in the baptism register
Please give the full name of the child's father, including any middle names
Please specify how you would like the child's father's occupation to be recorded. If you no longer work outside the home, you can either specify this (using "father" or "house-husband"), or you can indicate your previous occupation.

How to Contact You
We need an e-mail address for you if you wish to complete this form online.
Please give us the phone number you would like us to contact you on.
If you have an alternative number we can use, please put it here.
Godparents
It is customary to have 3 godparents. When baptising a boy, you would usually choose 2 men and one woman; for a girl, you would usually choose 2 women and one man. This is, however, only a custom, and you can have more godparents if you wish. This form gives you space for up to 5, which is the most that there would normally be.
Please give the name of the first godparent. There is no need to specify their middle names.
Please give the name of the second godparent. There is no need to specify their middle names.
Please give the name of the third godparent. There is no need to specify their middle names.
Please give the name of the fourth godparent. There is no need to specify their middle names.
Please give the name of the fifth godparent. There is no need to specify their middle names.
I confirm that these details are correct; and I understand that this form is only to give the church the details needed to arrange a baptism. The final arrangements will need to be made between us and the vicar in person, and cannot be done on the website.
